fluttery knee cap

My right knee cap 'flutters' when it is slightly bent during exercise - there is no pain with it. - but I always worry that it's a sign that I should stop what I am doing. It doesn;t flutter when the leg is straight or very bent, as in a lunge. I have had my knee checked by an orthopedic specialist and he says while there are signs of long term damage in the knee, there is nothing fundamentally wrong with it. I stick to low impact exercise as suggested by him. Anyone got any idea why a knee cap would flutter like this?
