In a medical was as in a patient arriving at a and e with a blue heart
We used to call babies born with some heart defects 'blue babies' but I've not heard the phrase 'blue heart' in a medical sense.
Could it be that the heart had stopped or was starved of blood?

(I'm only guessing, the extent of my medical background is stalking Sqad.)
As far as I'm aware the exact term does not exist medically. If a patients skin and mucous membranes are seen to be cyanosed they can be referred to as blue.

A blue nose, now. In NI and Scotland it describes a Rangers supporter.
Does it mean not enough oxygen being circulated?

I suppose it could be an informal term for cyanotic heart disease.
Unless you mean blue blood?
