A friend of mine had to be operated on for appendicitis at about the same stage of pregnancy - one baby, though, not two. The doctors and surgeons took meticulous care of her. She recovered completely but had a rather awkward scar as a result.

depends who it is and what the signs are

This is quite a difficult surgical issue - not when they think she MAY have appendicitis but when she has it and is pregnant

The surgeon and obstetrician will put their heads together and come up with a treatment plan which they will discuss with the mother

whose first question will be: will I lose the twins ?

and here is a technical paper on it: http://www.ncbi.nlm.nih.gov/pubmed/10535336
sorry that was the answer to the question: pregnant and appendicitis

The answer to the question: "she is pregnant with twins and got guts ache" is completely different.
first thing to be established is if the ' pain/symptoms ' are appendicitis or pregnancy related .
Appendicitis in a pregnant woman is a surgical emergency.

If the diagnosis is credible she will have an operation.
Abdominal surgery during pregnancy has it's risks but with a good surgeon these will be minimal.
My eldest sister had ovarian cyst surgery at Scarborough hospital in the late 80s when she was around 16 weeks pregnant and all went well, she had a normal pregnancy and gave birth vaginally at full term.
