Works for me. Fourteen questions. But I'm unsure the options always make sense. For example 'yes', 'no', 'don't know', when the answer is 'sometimes' ? Or a free hand answer for 'how often' rather than ticking a range ?

Oh and no one is going to want to copy & paste further links to answer. This is voluntary not paid for, it needs to be as easy as possible.

OG, the link in the OP works ok, it's the links in questions 7 to 14 that don't work!
