Hey there doc, how's things? Still being a cantankerous flirt?

I'm hoping you may be able to diagnose what a patient of mine was suffering with, as I haven't got the foggiest.

Male in his mid 50's. No significant history. Normally fit - regular gym and exercise. Sudden onset of abdominal pain radiating into his back and groin. Tender to touch in both centre an left side of abdo, with muscular guarding, and very sensitive to touch on his back around the left kidney. No abdominal distension or pulsatile masses. Pain coming in waves lasting about 10minutes each wave (pain 10/10); relieved posturally and with entonox. No problems with urination, and no pain on passing water.
Pt also complaining of unusual stools. Since problem started he has been feeling very constipated, despite successfully passing solids several times a day. Stools varying in consistency from very loose and watery, to 'rabbit droppings'. Bowel sounds were fine on auscultation.

My initial instinct was Kidney Stones or Renal Colic, but that doesn't explain the poo-ing problem. Very grumpy ED staff wouldn't tell me what was wrong when I went back later.

It's a mystery to my underdeveloped mind. Hopefully you may be able to shed some light.....

why would the ED staff not give you any information re the patient ?
I am also surprised that they would not tell you the diagnosis - unless they were totally clueless.
