Have to go soon with first to the hygienist /sadist and then to the dentist, any ideas on how to relax and how to distract your mind from all that is going on in your mouth! I think I have a low pain threshold and am just dreading it.

why dont you pay for a bit of sedation - the hygienist is dreadful - I once got 7 injections on one side of my mouth one Friday to get that side done and 7 the next week - please get the sedation - it is worth every halfpenny.
Try imagining your dentist in a pair of sexy trunks - or even naked. Of couse he has to be not bad looking to start with though.
Failing that - Close your eye's and imagine you are on a desert island, Dolphins playing in the surf, tropical birds singing, lovely blue sky above - works for me every time.
Forget the hygienist - get yourself a Sonic toothbrush, dental floss and those little wooden sticks to clean in between teeth. Unless you have a problem with your gums, like bleeding or have neglected your teeth, then by all means keep on seeing the hygienist.

I find it a nightmare and a waste of money, only to be told 'your gums are in good health'.
