Tit for tat I know but lets say a woman had a spiral fracture to her arm, she says it's caused by someone twisting her arm until it breaks. The person who did it rightly or wrongly is convince (or has convinced himself) that he just held her there and she twisted until it snapped. is that even possible, the latter, that is????

I dont think so; one usually tries to avoid pain not enhance it.
I would say that is possible.
IMO the only way it would be possible is if she had osteoporosis. She would tear muscle and tendon before the bone snapped and that is agonisingly painful
unlikely she would have the strength to do it before the joints dislocated. then she would no longer be able to apply leverage.... unless she was very strong or had an underlying med condition... which she may have needed anyway for this to happen.
