On the registration form, it says 'THIS INFORMATION IS REQUIRED BY LAW' - so if I tried to evade jury service by not filling it in, would I be comitting a criminal offence?

I thought that you could be fined upto �1000 for not completing it but Ive never heard of anyone being prosecuted. You need to be on the voters roll to get credit at good rates so be a good citizen and complete the form!!
