he woke up from the coma and realised he prefered life in the 70's to life in 2007. He then jumps off the top of a block of flats and wakes up again in the 70's. you are unsure if he is dead or in a coma again. He gets the girl and continues with his job.
Thats about it really.
We are led to believe that he wakes from the coma in 2007, but, at a meeting, somebody draws his attention to the fact that he has been idly cutting his finger with a piece of metal without realising it. He remembers what the barman in the 70s said - if there's no feeling, it's not real - so he knows that 2007 is an illusion. He jumps off the building to end the illusion and 'wakes up' back in the 70s. What is really hard to explain is how he knew so much about the future - mobile phones etc. Well, that's what I think.
Have the box set.....though personally prefer the idea that he is alive and well and living in the 70's he actually was from 2007 and commits suicide.
That wasn't it, Bert..what the barman in the 70's actually says is that if you can't feel you aren't 'alive'.

Sam was pysically alive in the present day, but not 'living' as it were. He was in a boring meeting at work instead of chasing criminals with Hunt & the gang..an Annie wasn't in his life.

This is why he decided to put himself back into the coma and go back to Annie et al.
Forgot to add that I think the show was trying to say that when you die or are in a coma you go into a parallel universe.
There's a link from a previous question on this subject which will take you to an interview with Matthew Graham in which he explains his view of the ending.
