Safely Remove Hardware

The icon keeps disappearing along with the volume icon. I go into control panel - sounds & audio devices - & click to add volume icon to toolbar, when I do that the usb icon appears. When pc next starts up the icons are gone. I've right clicked the toolbar & it's locked, how can I get the icons to stay there? In properties, the 'hide inactive icons' is unticked & I can't select anything in customize apart from 'hide when inactive'. I've tried it ticked as well but still no luck. I only started having this problem recently, it used to be fine.
