Don't you just have to show the registered LPA?

I am asking which particular form I have to show. All of them, some of them, a particular one?
oh.....My very old experience was that it varied from company to company....like death certificates.....ask the company concerned.
they are all jobsworths
and expect them to say
oh we dont want ...... we want......[ chose at random from A,B,C, X Y Z]

and oh we use a form D for dead you know the one they stopped issuing free years ago, because people said they werent ( dead )

shootthem all
PP that was pretty much our experience...oh and the best one when asked what form(s) they wished to see/receive copies of...was the confused lady who said "I have asked the manager and he doesn't know" as though that would end the matter. Sis, who was dealing with it, said "oh that's fine I won't pay the bill then" which, as they say, focussed their attention.

Not really, Buenchico, as that doesn't show the name of the attorney.
I have that form and many others. I shall be very glad when this rolls out to me
