-I'm sure someone will correct me
- but heading from Patchway onto M4 East means keeping in right hand lane. The left hand lane applies rather late to M4 West (google street view shows good view.I've looked at google maps and rac and they say keep left hand lane - when surely it can only be right -hand lane ? http://www.theaa.com/routeplanner/index.jsp#fromNode=0%7CAztec%20W,%20Almondsbury,%20Bristol%20BS32%204TS,%20UK%7C%7C-2.568684%7C51.541952%7CtoNode=0%7COxford%20OX33%201LJ,%20UK%7C%7C-1.097169%7C51.737997

sorry link doesn't work
Assuming that we're looking at the same junction (or, rather, fork), then the right-hand lane appears to be correct:

Unclear how you are getting on the M4. If up the road from Aztec West, you turn right on the roundabout and up the slip road.

Or you could go down Gloucester Road, onto the ring road, onto the M32 and then onto the M4.

Or is this a discussion about getting off the M5 and onto the M4 ?

Exactly ! - yet route instructions say
"Keep left at the fork to continue on Exit 15, follow signs for M4/London and merge onto M4"
The latter needs care until you're familiar as it's too easy to get off the M5 and then find yourself getting back on it.

Yes its -"Aztec West, you turn right on the roundabout and up the slip road."
IIRC that puts you on the M5. Just follow the signs, you'll need to leave the M5 to the left (naturally) then stay to the right *but not always far right or you'll end up on the M5 again*. Shortly after a Y juction splits into 2 lots of 2 lanes, there you need one of the right fork lanes to go to Bristol/London, the left ones to Wales.
To be honest, don't rush, be aware of the hazard potential return to the M5, and you should be able to work it out.
After you join the M5 at Junc 16, the signs on the overhead gantries will be in the following order:-

Get into the middle lane and follow signs for M4

At Junc 15 (first "split" between M4 and M5 = Midlands & Gloucester) bear left and follow signs for M4

http://tinypic.com/r/2z89ici/9

At Junc 15 (second "split" between M4 West and M4 East)bear right and follow signs for London Bristol M32 & M4 (E)

Hope this helps?
i think you have to stay in the leftest of the right lanes when they spilt and i think that's what it's trying to stay
