At a bit of a wild guess (because I've never seen a window like the one you refer to), I suspect that you might possibly have the 'Mouse Keys' function enabled. (It allows disabled users to navigate around their screen without a mouse).

This link refers to Windows 7 (but things are probably fairly similar with other versions of Windows) and tells you how to enable the feature. Fairly obviously though, you follow the same route to disable it:
