a female stunt person died on the set of deadpool 2, because the scene required that she wan't to a wear a helmet, sadly she died of head injuries.

Surely if
///she wan't to a wear a helmet///
she should have refused to do the Scene without one.
//she should have refused to do the Scene without one.//
Had she done so, she would have found it hard to get further work as a stuntwoman.
She also wasn't an experienced stunt person, she was a racer and this was her first film, which begs the question of if it's ever a good idea hiring people slightly outside their normal remit to do stunts.
There's more to this story, but no mention of the absence of a helmet being responsible for her death.
