From the AA website:
USA: IDP recommended as, in the event of an accident, some insurance companies will insist on seeing one. Additionally those licences which do not incorporate a photograph need to be accompanied by photographic proof of identity.

An IDP is available from the AA, or from some Post Office branches:

Chris
I did this earlier this month and all I showed the rental company (Alamo) was my credit-card-style UK licence with photo on it. Apparently I should have had the paper one as well, but I didn't know and nobody asked for it.
You can legally drive with your UK licence - old or new style.

Some car hire companies may insist it's the new style (and technically, should see both parts, but may not bother).

I've always used my old style UK licence with passport as ID - it;'s always worked so far
I've always just used my new style driving licence. You are supposed to have both parts with you though, and to carry the paper part with you whilst driving in case you are stopped by the police.
