I don't think you can. I'm pretty certain that you have to apply for a replacement passport (assuming you are in the UK).
Depends on your nationality. Some (eg the UK and most EU countries) issue a new passport. Others (eg the US and a lot of Asian countries) add extra fold up pages (a colleague has a passport like a brick now).

If you live in your home country, approach the passport issuing office. If you live overseas, you will need to go via your embassy. Normally the Government will have a web page that explains everything.
Assuming that you're UK passport holders, there is no way to get additional pages without applying for new passports. You can have 9 months of the unused validity of your existing passports added to the ten year validity of your new passports but, beyond that, there are no special concessions.

A new passport will cost you 66 by post. If you use the Post Office 'check and send' service, you will pay 73. Making an appointment at a passport office, with your new passport being sent within a week, will cost 91. The same-day service at a passport office costs 108.

However, those prices are for a standard passport. It seems likely that you will require 48 page passports. The fees are, respectively, 77, 84, 97 and 114.50.

Just to add to Chris's post, the prices quoted for the various means of application increase on 4th October to:

32-page... 72, 72+handling, 97, 114
48-page... 85, 85+handling, 105, 123
