Cinema Card

I have a CEA CARD,for use in the cinema,it has expired and I am trying to renew it by post,it says on the back of the card to go to www.ceacard.co.uk,which I done,to renew by post I have to download the form but I cant do that,i am not good on the computer,what can I do,any suggestions ?

after several phone calls I have been told they will give me a form to fill in at the cinema
