What Voice Disorder Could This Be?

I think there is something wrong with my voice.
- I've struggled with chronic coughing/clearing of the throat for the past 7 years. It is usually worse when I am nervous or anxious. Once I start to clear my throat I cannot stop.
- My voice is fairly breathy (especially my head voice), but not hoarse.
- I have a weird section in the middle of my voice when I sing. My voice goes out and it is impossible for me to control and find my tone. I become extremely breathy, or I loose my voice completely. It is like vocal break/bridge/passagio but much much worse. And it will not go away despite extensive vocal exercise and training.
- When I get a very bad cold I loose my entire head voice completely, but my chest for is totally fine.
- It is extremely easy and effortless for me to hit high notes, going above f6. ( I literally have no straining or tension at all. It stunned my old vocal teacher).

Do you mean you are constantly aspirate? If so you need to better anchor yourself from the diaphragm. What is your complete range and how old are you please?

@kvalidir I am not sure what "aspirate" means. My complete range is D3-A6 and I am almost 20 years old. (I am female)
Aspirate is when the breath passes through the vocal folds before they begin to vibrate, giving a breathy sound. How are you trying to use your voice when this happens and what sort vocal teacher do you have and what method do they teach to please?
Throughout most of my teenage years (and beyond) I was constantly clearing my throat (to the point where my teachers complained about it), often coughing and experiencing occasional voice problems. The cause turned out to be laryngopharyngeal reflux:

So you might find that taking proton pump inhibitors could solve your problem.

@kvalidir It feels like I am letting supported are from my belly flow out, but once it passes through my vocal chords and nasal mask I no long have any control over it. My voice teacher once said I sound like a boy in puberty in that area. It feels as if it just does its own thing, and I cannot control its sound. It also feels like I have no support, despite taking a deep breath. I used to take lessons from a classically trained opera singer who strictly taught classical and bel canto techniques. I ended up stopping because I've been too frustrated with my voice, and it was causing bad anxiety.

I was actually diagnosed with that by an ear nose and throat doctor when I first started have these symptoms. He said there was significant signs of damage caused by acid reflux on my vocal chords. However, I then saw a gastroenterologist and got a procedure done. He said I actually do not have it because there is actually no acid coming up from the stomach.
