How does a community social worker access various local authority services for the client (eg: community care workers, provision of day and residential care etc)

I'm studying a course (NOT social work) and need to know this but...I've googled etc and no luck

PInkwink and saxyjag were very hepful a few days ago with a different social work Q

PS - what i THINK happens is:
1) REQUEST for assessment

2) ASSESSMENT CARIED OUT by social worker (taking into account the wishes of person being assessed. Their physical / mental difficulties. Health / housing needs. Current sources of help (family, carers etc) / The needs of these sources of care.

3) A CARE PLAN is Produced (stipulating service to be provided - when and by whom / Contact point for problems / Information on how to ask for review if circumstances change.

Does this sound about right?

Confine your interests/requests to a particular LA's Social Work Department.
The more you keep it local and the more you get to know each other's faces, the greater fruit will be borne from it.
