Really pulling my hair out ! How do I prepare indexed paginated bundle for trial?

paginated just means numbered pages dosent it? so just get everything together, number the pages then do an index (ie a page at the front which say something like:)
background info pages 3-7
medical report pages 4-12
witness statements pages13 - 18
Yep bednobs is right. If it is fast track or multi track it should be agreed with the other side. So really you should do an index.

Normal trial bundles should be in this order:

Pleadings
Court orders/directions
Witness statements and exhibits
Other correspondence

You should do four copies - one for you, one for the other side, one for the Judge and one to go in the witness box.
