When 'k' is added to mean 'thousand', for example kB and km, is it meant to be a capital 'K' or a small one?

I am pretty sure it is lower case k but have no proof.

yeah, as far as I know, the capital K only came in with computing terms to distinguish 1,000 and 1,024 multiples of kilobit and Kilobyte.
