removal of name on birth certificate- someone please help!!!

My ex partner and I have undergone a DNA test on my child and the results showed that he is not the biological father. I know I should have done something earlier , but I didn't. I have not spoke to my ex partner but I am sure he would like his name removed off my childs birth certificate, as would I. I have no idea how to do this, I have searched the internet and found little information...someone please help and tell me how I go about doing so.....thanks
