if  a  person  wants  to  die  in  a hospice  do  have  permission  from  gp  or  make  a  donation

The GP or hospital refer patients to hospices. No donation is necessary.

^^^ With some hospices, patients and/or carers or relatives can also make their own direct referrals to hospices.  e.g. from our local area here in Suffolk:

^But in that instance the hospice would communicate with the patient's GP to ascertain medical details. 

Just remember it depends on what condition you have, many hospices only accept patients dying of cancer, motor neurone disease, or sometimes MS. If you are dying of say COPD, renal failure, heart failure for example they are unlikely to provide end of life care.

Admission to a hospice also depends on what beds they have available at time you need it, And how long you are assessed to have left to live. Also, if the patient doesn't have any difficult to control symptoms they would not normally Be admitted. As has already been mentioned it also depends on the diagnosis usually
