I can think of no surgical procedure that requires routine post operative anti coagulant therapy. Surgical procedures on heart valves are routinely given anticoagulants BEFORE surgery, but in other situations anticoagulants are used for the treatment of certain post operative complications.e.g DVT or perhaps abnormal heart rhythms due to various reasons.

Have you got anything in mind?
Sorry, I am sure that you know that Innohep is an anticoagulant.
