Hiker's Heat Rash

In June I am about to embark on a long distance walk over 24 days. When I did this before many years ago I got a heat rash which ended up with me having really painful legs and having to go to the Doctors. I am keen not to have this happen again! I think this is quite common in older people. Does anyone have any tips on how to prevent this happening? Thank you in advance.
