When rice has been cooked does it needed to be refrigerated or can it be left out and covered.

I would put it in the fridge and use it up quickly or freeze, its one of the highest causes of food poisoning
I usually leave it out for the day, until everyone's eaten for the day, then refrigerate it.

I usually leave cooked rice in the fridge for over a week and it's always good, and I've not gotten ill from eating it. If it's not eaten after a week I freeze it.

My Mum used to cook huge pots of rice and left it covered on the hob all day and no one got ill from eating it. Everyone grew up to be healthy men and women, never got sick once from eating rice left 'outside'.
just make sure the rice has cooled right down before you put it in the fridge, it needs to be covered too. and shouldnt reallly be used after 4 days. when reusing the cooked rice after 3 days i would recommend to pour boiled kettled water over the rice before use.

Think you have just been lucky,rice and kept gravy can be major problems
