Dog Tags

Hi, can anyone tell me what information is required on a dog tag in Scotland? I currently have my dog's name, my address and telephone number. There has been an increasing amount of assaults on dog walkers in my area and I am worried about giving too much info, but want my dog returned should anything happen to her.

^ Yes not a tag which can be removed, dogs need to be microchipped.
The microchip just gives a reference number which can then be looked up on the database which is held by vets, police,animal charities etc.
