When I visited my doctor after getting diagnosed with cancer,he gave me a form to fill in,I filled the few questions that were relevant to me and he filled the rest in,the doctor said it would be more cash for me,I told him I was on attendance allowance and he said its got nothing to do with that,the form is DS1150,anyone know what this is please

razza, have you got the right code there? Googling DS1150 brings up results for a docking speaker. Ive never come across a DS1150 form
There is a form called a DS1500. What is your prognosis?
Is this question definitely about you and not someone else?

If the DS1500 is the correct form it does not necessarily mean extra money for you, razza, but the GP does get paid for filling it in.

Yes it is about me,I told the doctor i was on aa,the form number is correct,my doctor sent me a copy,I am on the higher rate aa,Ive googled it a lot and cannot find a definitive answer,they have given me 2 years,cancer in 1 kidney,a hole in my pelvis and a shadow on my lung

Get in touch with Macmillan nurses, even if you do not want the nursing care they have specialist benefit claim advisers who know the system inside out. They will find exactly what you are entitled to and make the claims on your behalf. They also have direct access to the various government departments that supply the benefit via special dedicated phone lines that are not available to the general public and can get the payment made a lot faster than you doing it yourself. It is completely free to use them and they are very helpful and understanding.
A close friend and neighbor was recently diagnosed with cancer and they were able to get his first payment in just 5 days, It was also a lot more than he thought he was entitled to!
You have been given a form we have not heard of if you are adamant it is a DS1150 and not a DS1500. I know what a DS1500 is for but if it's not that, I suggest you 'phone your doctor to ask what you're meant to do with it.

Contact McMillian, they are very helpful. When I was diagnosed, they spent hours on the phone with me helping me fill out forms, they even did it in stages (half hour at at a time) so I didn't get too tired.

I have not claimed it myself,my doctor claimed it on my behalf,I filled in part of it,I had to take it back to the doctors and thats where my involvement finished,apparently he then fills the rest in,diagnosis,prognosis,then sends it on to whoever.The form said on it,this is not a claim form,but the doctor said it means more cash for me,would be very nice but I would just like to know where it comes from.Eddie51 looks like he could know if he asks his neighbours friend
I don't know anything about the form Razza but I wanted to say I am sorry to hear that you have had this diagnosis.

Corby knows about things like this, but the name for your form (DS1150) doesn't match any of the forms for AA.

I think you have mis-remembered the number of the form and I am sure it is a DS1500 and is used when Special Rules apply. If you are on the higher rate of AA already, your rate will not increase.
razza has a copy of the form with the number on - she has not misremembered.
RAZZ, if you have a copy still, what does it say on the top of the first page and how many pages does it have?

Thanks to all for trying,I will just have to wait till I either get it or dont get it,whichever way,I will let you know in case it comes up again in the future
