Lost Passport

A family member has had her passport lost whilst it was in the possession of the Passport Office in Glasgow, the envelope apparently only contained (according to the Office) her certificates and money. They took the payment and just did nothing about it until she phoned up to find out where it had got to. They told her she had not put the old passport in the envelope, but the certificates and cheque was inside the passport - therefore the whole thing must have been received by them at the beginning. She has had to fill in a Lost or Stolen form, and notify the police, fill in another passport application and send that together with more photos.

Has anyone else had a passport lost whilst in the process of renewal at the Glasgow Passport Office?

Maybe the passport was stolen while in transit and envelope resealed and they really only did receive the certificates and payment.
Quite the reverse happened to friends they applied for visas to India ad when their passports came back a third passport belonging to a man in the Midlands was inside too, they rang the contact number in the back of the passport and spoke to the owner, they a should they post it on to him ?he said no fear it might get lost again and drove all the way down to Suffolk to collect it in person.
