Do I need Visa for months holiday in USA and how much does it cost, or does visa waiver cover it, and how much is that?

Provided you qualify, all you need is an ESTA, apply here $14

By qualify, it's regaring criminal records, arrests etc, so have a read of the website.

If you don't qualify for the ESTA/visa waiver, the fee is $140, payable upfront when you schedule your appointment at the Embassy. You may also have to produce a copy of your police record.
