What Happens When A Primary School Is Investigated For Maladministration Of The Ks2 Sats?

What is the involvement of the local authority?
What responsibilities does the SLT have in regards to keeping the teachers on staff informed of the process? (teachers that are not involved, who have deduced that this has happened but been told nothing and too scared to ask!)
Does this mean the school should shortly expect an Ofsted inspection?
If there is evidence of maladministration who should ultimately be accountable for this? (The executive head teacher or the newly appointed head of school?)
