What is the song and who recorded it?

This track is being played a great deal at the moment on TV, particularly in relation to moments of triumph over adversity. It was used during the 2008 Paralympics, and the BBC use it quite a lot (Extreme Dreams, etc). I have heard a short clip of the vocals, but it sounds like the words are not English (maybe in Swedish???), however the sample used is almost always instrumental.

Hope an ABer can help, it's driving me mad not knowing!
