My 1998 S reg Audi A4 has been sporadically overheating, it has blown one of the head gasketss. What can cause this to happen?

Insufficient coolant, faulty thermostat, blocked radiator or restricted grille, cooling fan faulty, airlock in cooling system or expansion tank pressure cap faulty. Or something else.
