Driving license form

I have moved house & need to update my driving license - however, I cannot find it any where now (I haven't actually seen it for years!). Does anyone know what form I need to fill in in order to replace license & change address at same time? And what this will cost?

Tried emailing DVLA but got no response.

You can get a form but it will cost you for a replacement, see here ;-

You don't have to pay for change of address only, you can do it online. Do you know your driver number? If you do you may get away with not having to ask for a replacement due to loss etc.
You will need to send them back your old licence and if you haven;t got it you will probably need to pay. You will also need to send off an up to date passport sized photo as well.
