I am trying to find something on this on the internet without much joy. A sample policy would be great.

I don't know about this, but suspect it would help if we knew the context you need it for.

Should have made this clearer. It is in the social work context ie carrying out assessment of needs and then drawing up of support plans. It is a requirement of Supporting People.
Any clearer now?!
