1. If the main drum bearing has developed end-float .. which is unlikely .. the belt will come off (will usually be noisy on bearings tho, when drum rotating)
2. Drum pulley centre bolt thread stripped or loose .. allowing drum to move out of alignment with motor pulley. Check and tighten. If thread stripped and bolt will not tighten, it will need a thread repair/insert or a new drum (never seen this happen tho)
3. Drum pulley cracked or buckled (not unheard of) .. replace pulley if cost of part not prohibitive.
4. Motor attachment bolt loose, missing .. or belt very loose. Check and rectify.
Belt tension is by pre-load spring OR by adjustment arm/bolt (often 17mm)
