I recently boiled milk in a pan before adding the hot chocolate powder and boiling it again but the second time that I boiled it with the powder, it started to evaporate and made a mess.

So how many minutes for each boil? or should had I have simmered it instead?

Simmer...I'd whisk the cocoa+ sugar in a saucepan with some of the milk, then gradually add the rest of the milk. Heat gently.
I usually put the chocolate powder into a cup, then add a little milk, then mix into a paste. Once the milk is hot enough, pour into the cup. Milk tends to cool quickly, but it can be re-heated in a microwave.

It's the first time I've ever heard of anyone putting the chocolate powder into the pan
I've done it both ways. I think the cocoa powder dissolves a bit better done in the pan...but if I'm in a hurry it's all in the mug. ;-)
If its the Cadburys or Ghirardelli type hot chocolate then I whisk or stir it into milk in the pan as the milk is heating. Once the milk starts to move around the edges of the pan then I remove from the heat and serve. I never ever ever boil milk for any drink even once, let alone twice because I think it makes the milk taste disgusting. If its one of the instant type mixes, then I would dump it in the mug, pour on a little hot milk, mix then dump in the rest of the milk and stir like crazy.
I'm with woof...why on Earth would you boil the milk? Even instructions on cocoa powder say to mix powder to a paste with a little cold milk, then add HOT milk and mix....no mention of "boiling" or "boil".
No wonder it caused a mess...you were on the way to making evaporated milk!
