Income Tax Check List

I have an accountant who has always done my paperwork. However last year he sent me an Income Tax Check List, which basically asks me to fill out all my interest, benefits, any income which I have received. I ignored this, as I felt that I am paying him to do all this. He did my paperwork as usual last year. This year he has sent me another one to fill out. Is it my responsibility to fill out this Check List or is it something he should be doing? Thanks all

he will do it and charge
I have one and try to do all the book keeping myself
to save the charging time and fee

by and large it is expensive to pay an accountant to keep books

[ and hey I am currently doing AAT level 2 book keeping ! - clearly taking my own advice seriously ]

He charged me the same last year even though I didnt complete the checklist, so I wont do it this year either. My accountant is for peace of mind. Thanks for the help Peter.
