I have friend has been given a very strange tax code despite working 16 hours part time on the minimum wage.
So I looked into the matter and what I have discovered is the her NI shared with another person with the same first and last name. Only differ by the middle name. The other person owns a business and hence self employed.
This probably explains why the tax code is not the £10,000 tax allowance code.
We have contacted the tax office and they have ignored my friends concern. I think the tax office does not know how to fix it. This potentially affects the pension in later life.

Any recommendations to resolve the problem is appreciated.

To be quite honest this doesn't sound possible. It certainly won't be 'ignored' by the taxman. (perhaps they're investigating your friend's part in some kind of fraud)
HMRC the tax office deal with taxation. The Department of Work & Pensions (DWP) deal with NI payments. Write to DWP address is in Newcastle
I think newbie is saying they have the same NI number.
^so the best approach is to contact the DWP who should investigate and then issue a new NI number.

Yes, I am saying two people using the same NI.

A few years back my friend contacted CAB to write to the relevant department and had a a letter saying it has been sorted out. During that time my friend was a house wife and did not work, so she never been seen any payslip to indicate that her tax code was still incorrect. It is just recently that she returned to work as the children has grown up etc.

It looks like the department that says they have sorted out has shoulder off the problem.

If two people contributes to the same NI , I don't know how they will fix it , in terms of who and how much is contributed by the individuals. Especially when the IT system and has changed over the year ( 30 or more). So for this reason they have ignored the error.
My friend only noted that when she was claiming child benefit for the kids and the middle name was of someone else. At the point the name was corrected, but implies they corrected on one IT system but not the others. Hence now the problem still exists.

and yes it can happen and she needs to sort it out now and not wait until she is 65.
