I return to my current job a month ago after being on maternity leave for 9 months. I have been suffering from postnatal depression and anxiety since having the baby and although it’s two days a week I’m finding the job too stressful. I have found another job which I think will suit my current situation better, how should I tell my boss? I feel really bad they have no idea I want to leave. Should I tell her the reason or say something else? Im quite a private person.

Its up to you but I would tell the truth...I mean why wouldn't you?
Honesty is the best policy. Just say a career change has presented itself which you wish to pursue and thank them for the opportunities over the time you were employed.
If you don't feel able to discuss this face to face, your resignation letter can cover your feelings. On the one hand, it's a formal statement; Dear X, I would like to tender one week / month (whatever) notice of my intention to resign. (If you like you can say 'with effect from X date, just to make it clear). (but then you can be a bit more flowery and explain things) Having been presented with a new opportunity that will suit my changed circumstances better, I'm moving on with some regret. I'd like to take this opportunity to thank you for your support during my time at Bloggses. yours sincerely.

It's really not necessary to give a reason if you don't want to, choice is yours
Dear........Please accept this as formal notice of my resignation from the position of ....., with effect from.......

In accordance with my contract of employment I am happy to continue to work until ......

After being back from maternity leave, I have found it difficult to confidently comprimise my time between family life and work. Another opportunity has arisen to which I feel will meet my needs accordingly. I am sorry to leave, and I thank you for your support during my time with the company, which I have found enjoyable and fulfilling.

I would appreciate a further discussion to detail my personal situation in more depth.
