4 weeks ago i slipped on a grassy bank and twisted my ankle over on its side, immense pain, just about got back home with the help of 3 large dogs to lean on, only 200 yards.

Iced and elevated it immediately and got to casualty within the hour, they said it was badly sprained and gave me tubigrip to support it. I kept it packed in ice and elevated until 5.30 then I had to go to work (I had to go, new job)
I struggled walking and working for 3 weeks. the last few days it has got a bit better but still aching and swollen but now it has stopped improving, its swollen mostly around my Achilles tendon and there is a definite lump on the tendon, possibly from previous Achilles tendinitis, also swollen on the side of my foot where it was sprained Should I still be icing it and elevating or just leave it and let it take its time, or maybe see a physio?

I know I should have taken a few days off work to rest it, I hate taking time off work, especially when I have only been in this job a few days, I think I am now paying the price,

Yup RATTER you are now paying the price. In your circs, I'd be seeing a physio and getting advice AND TAKING IT :) R.I.C.E might still be enough to fix you but the swelling might benefit from some treatment to dissipate it and I wouldn't rule out a touch of arthritis or tendinitis. If you can, find a physio who specialises in sports injuries.
