cabin pressure is lower than sea level pressure
your ears, nose and throat all allow the pressure to equalise (the same pressure which pops your ears), or whatever
if any of the ventilation in your head is blocked - the pressure can't equalise and it will cause discomfort or pain.

I tend to get a throbbing left temple because the left tube in my ear is narrow - it does pass though (for me)
